Dementia Information and Advice Service - Engagement Survey
Overview
Help us improve the dementia information and advice offer in Warwickshire
Warwickshire County Council currently buys an information and advice service from an external provider who offers this service to anyone affected by dementia, including carers, who are living in Warwickshire and are registered with a Warwickshire GP.
The Council is conducting a survey to understand what people need from local dementia information and advice services, both now and in the future. The feedback from this survey will help the Council improve these services to better meet people's needs.
Your responses will be kept confidential by the Council. The summary of the survey results will be used to design better dementia information and advice services in Warwickshire.
