Neurodevelopmental Diagnostic Service and Support - Survey for People Awaiting an ASD /ADHD Diagnosis and their Parents / Carers (Residing in Coventry and Warwickshire)
Overview
We are seeking the views of people awaiting a neurodevelopmental diagnostic assessment (ASD and/or ADHD), as well as those of their families and carers to help us to design the best solutions to this challenge.
The purpose of the survey is to understand more about:
- the reasons why you or your family member are seeking a diagnosis;
- what support would help you or your family member now while you are awaiting an assessment; and
- your experience of support and services you have accessed and whether these have been person centred and effective.
We are soon to start a Stakeholder Group for people with lived experience of having autism/ADHD and/or their families and carers. A stakeholder is someone that has an interest in a particular subject or group.
The group is to develop and embed a coproduction approach to what and how various support services are developed and commissioned in Coventry and Warwickshire.
If you would like to find out more about the group, please contact us at disabilitiescommissioning@warwickshire.gov.uk

The following survey is for individuals and their parents/carers who are waiting for a neurodevelopmental diagnostic assessment only.
There will shortly be a different survey made available to professionals.
