Please explain the rationale for the judgement 

  • Recording shows appropriate system checks being carried out (Mosaic, CF6 and Sentinel Database) 
  • Initial liaison with Police is recorded 
  • The case recording (and closure note, where appropriate) enables progress of the case to be understood 
  • Consider compliance in line with statutory timescales such as strategy discussions, decision to initiate assessment/ S47s.  If strategy discussion held, consider quality of this, analysis and actions.

  • Consider referrals received within the last 12 months. If the intervention exceeds a 12-month period, then referrals outside of this time frame should be considered.
  • The reason for the referral/s is timely, clear, and linked to the identified concerns.   
  • The referral sets out the reason for the referral and required outcomes. 
  • The history is considered, and action is taken to ensure the correct level of intervention is provided. 
  • Referral workflow is fully completed  
  • Evidence of LADO advice being provided to referrer within 24 hours of referral, including plan for LADO oversight, as required 
  • Initial threshold decision and advice to the employer is recorded with reasons and plans for reviews communicated as necessary 
Assessment

  • Minutes capture relevant detail and decisions and are shared in line with appropriate timescales 
  • Focus on evidence and investigation is clearly recorded with rationale 
  • Initial and subsequent planning addresses child safeguarding considerations to include needs, harm and risks 
  • initial threshold decision draws on available information and inform advice to the employer/referrer
