Recording shows evidence of jargon free language & usability, the use of professional assessment of recording, ESCR and Mosaic usage

Childs Journey/ Carers Journey is up to date, contains significant events, themes and is impact based. It is recorded on the “Child’s Story/Carer’s Journey” form within Mosaic with evidence of “analytical overview”

Consider referrals received within the last 12 months. If the intervention exceeds a 12-month period then referrals outside of this time frame should be considered.

  • The reason for the referral/s is timely, clear and linked to the identified concerns.  
  • The referral sets out the reason for the referral and required outcomes.
  • The history is considered and action is taken to ensure the correct level of intervention is provided.

Any re-referrals should be considered to explore if assessment/ intervention was appropriate or if there was gaps in understanding of need.

  • Consider compliance in line with statutory timescales such as strategy discussions, decision to initiate assessment/ S47s.
  • If strategy discussion held, consider quality of this, analysis and actions.

Carers have a high-quality, holistic assessment that is timely, comprehensive, analytical, identifies and manages risk.  It considers family and social relationships including contact, identify, development of self-care skills, suitability of placement and accommodation.  Pathway to Change Model is used.
