  • If threshold for LADO oversight met, the plan for oversight is recorded 
  • If no POT meeting is being held, the plan for investigation and review been communicated in writing to relevant parties 
  • POT minutes have been completed and sent out in 28 days 
  • Where the case is closed, is determination with a clear rationale  
  • Multi-agency working results in effective planning  
  • Meetings and reviews are timely, with the appropriate sharing of information and delegated responsibility. 
  • There is robust scrutiny and challenge of plans. 
  • The right people are at meetings, providing the right support, with effective partnership challenge when this is not happening.

  • The file shows evidence of relationships, feedback, high challenge/ high support and restorative working  
  • There is focus on identity, trauma, history/experiences, evidence of professional curiosity - challenge and support and consideration of research 
  • Planning appropriately addresses the risk of harm to the child.
